Two Rivers Health Department will be providing Flu Shots and COVID-19 shots (age 12 & up) to students and employees of Franklin Public Schools the morning of Monday, 10/11. Parents will be receiving a mailing that will include; a cover letter from Two Rivers Health Dept., a consent form, (which doubles for both vaccines, but you are able to choose what you want and don't want) and a Vaccine Information Statement indicated for the flu vaccination. AGAIN, THESE IMMUNIZATIONS ARE TOTALLY VOLUNTARY AND STUDENTS OR STAFF ARE NOT UNDER ANY OBLIGATION TO RECEIVE THE INFLUENZA OR COVID-19 VACCINATION.
Please return the consent form back to the school before Friday, October 8th if you would like your student to receive either or both of the Flu/COVID vaccination
